For the 2011-2012 school year we request you fill out the two forms listed (linked) below. Please return your completed forms to the school office before classes begin in the fall.
Please complete one copy of the Emergency_Information_2011-2012.pdf for your student. Please remember to select your information for the Fall Family Directory.
Please complete the Walking_Photo_Video_Permissions_2011-2012.pdf by circling your selections and signing the form.
You may complete and return the Student_Residency_Questionnaire.pdf, so the school understands students' needs.
