For the 2010-2011 school year we request you complete the forms listed (linked) below. Please return them to the school office before classes begin in the fall.
The Registration_Forms_Checklist.pdf may assist you in organizing and completing your student's registration forms.
Please complete one copy of the Emergency_Information_2010-2011.pdf for your student. Please remember to select your information for the Fall Family Directory.
Please complete and sign the Special_Needs_-_Expulsion_Form.pdf.
Please sign and return page 2 of the Technology_Code_of_Ethics.pdf.
The office would like current information from your student's last school. Completing a Request_for_School_Records.pdf will allow us to request the records.
Please complete the Walking_Photo_Video_Permissions.pdf by circling your selections and signing the form.
Please provide information on the About_My_Student.pdf form. Your responses are helpful to the teachers and other school staff.
It is helpful to have a completed Health_Appraisal.pdf.
Your family's proof of Michigan residency, a copy of your student's immunization record (or a completed Immunization_Waiver_2010.pdf) and a copy of your student's birth certificate complete the requested documents and forms.
The school may gain funding for students whose families are eligible for support. Completing the Free and Reduced Price School Meals Application documents eligibility. This form will be posted as the 2010-2011 income guidelines are available.
