Forms for New Students

To Families (Parents) of New Students:

Welcome to the Ann Arbor Learning Community!


For the 2008-09 school year we request you fill out the forms listed (linked) below and return them to the school office before classes begin in the fall. There is a Registration_Forms_Checklist.pdf available to assist you in completing your student's registration at AALC.


Please complete one copy of the Emergency_Information.pdf for your student.


Please sign and return page 2 of the Technology_Code_of_Ethics.pdf.


The office would like to have the most information about your student. Completing the Request_for_School_Records.pdf will help us collect records from your student's former school.


Please complete the Special_Needs_Expulsion_Form.pdf and the Walking_Photo_Video_Permissions.pdf.


Provide information on the About_My_Student.pdf form.

It is helpful to have a completed Health_Appraisal.pdf.


Your family's proof of Michigan residency, a copy of your student's immunization record (or a completed waiver form) and a copy of your student's birth certificate complete your student's registration.
